Welcome to the 2024-2025 school year.
Miss Darcy ( Darcy Noble, RN) is looking forward to assisting your child with their health needs. You can reach her in the Health Office 802-875-6846 or call the main office to be transferred. Someone is in the office from 7:30 am- 3:00pm. I will be performing hearing and vision screening in the fall. CAES will have access to dental care and education. Contact the office if you would like to have your child seen by a hygienist. There are registration forms to be completed.
A reminder to parents/guardians about the medication policies/chronic condition requirements.
All students receiving daily medications during the school day will need a written order from their Provider and parents to bring in the medication in the original containers.
All students with chronic conditions that may need medications for symptoms will need an ACTION PLAN from their Provider and parents to bring in the medications in the original container. ( asthma, diabetes, allergic reactions, seizures, migraines).
Students that need short term medications like antibiotics will need Provider notes or order, and parents to bring in the medications in the original container.
Parents can ask their pharmacy to separate medications into 2 containers. Your pharmacist can be very helpful.
Immunization requirements:
All students must have the required immunizations per the Vermont Department of Health to attend school. Please forward a copy of your child’s immunizations to the Health Office.
For students who are Religious Exemptions- Please forward a Vermont Department of Health Religious Exemption form to the Health Office. This needs to be completed yearly.
For students with health conditions that contradict receiving vaccines- Please forward a Vermont Department of Health Medical Exemption form to the Health Office. This needs to be completed yearly.
If you have any questions, please feel free to contact the health office. Forms are available at the Vermont Department of Health website, along with very useful information regarding vaccines and illnesses.
Please fill in the Power School Student Health forms as completely as possible with allergies, medications taken daily and as needed, dental and medical provider visits, asthma questions, medications that we are able to give in the school setting, and permission to discuss health information with your child’s Providers.
